Welcome to the Bitjam medical health app blog series. Last week we discussed the increasing number of apps available on the market, how HCP’s are dealing with conformance and compliance and the effects of app technology on interoperability in healthcare today. Part two will investigate the technology designed to help patients change behaviours based on Prochaska & DiClemente’s ‘Stages of Change’ model, and the difference between informing and changing – striking the balance between giving accurate and useful information
Many of you reading this article will have first hand experience of trying to change certain behaviours, whether that’s by exercising more, eating more healthily or spending more time relaxing. We know that these changes in behaviour don’t happen overnight, but over a period of stages beginning with precontemplation and ending in action maintenance. As we know, maintaining the changed behaviour so that it becomes normalised day to day rather than short term is very tricky, and in many cases relapses occur, that’s if the idea even makes it beyond the precontemplation stage.
The Stages of Change Model
Different to a theory, this is a model and highlights the various stages and states of behaviour change. It’s also known as the Transtheoretical Model.
- Precontemplation – the earliest stage of change consideration. It can take around another 6 months to take any action during this phase. More often than not people will emphasise the cons here rather than the pros
- Contemplation – also known as the recognition stage, this is when people start to see how adopting healthy behaviours could have an positive impact on their wellbeing and might consider action in the next 6 months, but are perhaps likely to still feel ambivalent to the idea of change
- Preparation – people are ready to take action within the next 30 days, perhaps by taking small determined steps to assist behaviour change. They start to realise that healthy changes could make them happier
- Action – people have recently changed their behavior and intend to keep moving forward with their healthy habits
- Maintenance – people have sustained their behavior change for more than 6 months and intend to maintain the behavior change going forward. This stage is where the work to prevent relapse occurs
- Termination – this is the final stage of the model, in which people are sure they will never return to their unhealthy lifestyles. This stage is very elusive, as most people will always stay in the maintenance stage, working continually to manage their healthy behaviour but still at risk of relapse.
How Technology Can Assist Behaviour Activation
The role of technology not only aids the initial action stage but can support maintenance and deal with potential relapses. As the model in figure 1 suggests, during the stages of change, relapse, exit and re-entering between Determination and Maintenance is a common occurrence. People need as much support as possible to stay – or return back to – the track.
Yet it’s not about how sophisticated the tech is, the key to success is in cracking the psychological obstacles indicated in the Transtheoretical Model, changing the mindset of the person and supporting then offering continual support during maintenance.
Technology is offering a new fix for one of the most confounding health-care challenges: getting patients with chronic disease to take better care of themselves.
This quote perfectly demonstrates the challenge faced by developers of digital health apps. As well as the technical complications – such as data privacy, regulations and synchronisation with older systems – the developer has to present the user (who will now be in the preparation stage”) with compelling language and data that will convince them to move forward into action. The developer will need to have a thorough understanding of the difference between informing and changing, depending on the intention of the app.
The Transition from Informing to Changing
Informing is simply passing useful information on to somebody that’s relevant and beneficial to their treatment, but changing is the act of using this information, implementing the learnings and actively applying the treatment in order to provoke positive and longer lasting behaviour changes. Changing is the activation of positive behaviours. The app must strike the balance between giving accurate and useful information in the most appropriate way and at the right time. This is why understanding the Stages of Change Model is crucial to the performance of a digital health app, the developer must know where their user currently sits in the model.
How Apps Can Actively Assist in Behavioural Activation
- They are more readily available to the patient, using device platforms that they are likely to carry around in their pockets
- The user has 24 hour access
- The user can monitor their progress, which can stimulate positive mentality towards healthy changes
- Accessibility promotes patient empowerment, provided the developer has also considered the challenges of accessibility to those with disability, lack of computer skills etc. Inclusivity needs also to be at the forefront of development

A patient with more than one condition – diabetes AND kidney disease for example – with several life-changing symptoms, requires them to take lots of medication that might have multiple side effects. Such difficult health conditions can be challenging to live with and some people find it hard to deal with the pressure, especially when patients are discharged or in-between services.
Combined Healthcare realise that many long-term sick patients aren’t simply somebody with one condition – they tend to have a number of health areas that they would like to manage better. This can be a frustrating experience for the patient, as often the information is not readily available, and they might also have to deal with polypharmacy if they are being passed to different services. It’s equally as challenging for the clinician, as they lack the time and resource to give the patient the full care they require.
This is why the idea for BeAble was born. A post-discharge system that provides the clinician with a library of more specific illnesses and treatments that aren’t just pulled in from NHS Choices(our research has considered this to be too long-winded and not specific enough). The library also provides the clinician with ideas for therapeutic activities to encourage a more active and happy patient. The patient is then provided with personalised information and a tailored plan about how to manage their conditions post-discharge, to stop relapse-prevention.
Bitjam are building two parts to the BeAble system – a mobile app and web platform from which clinicians will be granted access.
BeAble will have a number of core features, including a library of specific information and intervention suggestions for clinicians, written in a way that patients will understand, as well as encouraging patient control as they will have the opportunity to be more active about their condition (behaviour activation). Patients will have their care plan accessible from their mobile phones which will have the ability to connect with other technologies such as Amazon Alexa and Google Fit, enhancing the user experience by linking with apps that are useful to their ongoing recovery.
At the heart of BeAble is the idea of supporting patients who might be between many services or post discharge with the need to provide relapse-prevention support, to ensure patients are receiving quality and accurate care.
Patients will work more closely with their clinician to decide on appropriate therapeutic activities, which will help to establish and strengthen a relationship between clinician and patient. Any questions that the patient might have can be written in the app and up for discussion during appointment times. This is known as “channel shift” and it’s shown to improve communication. Patients can message as and when they need to. Channel shift could be the beginning of a move away entirely from the traditional appointment times as the idea is to reduce the number of necessary appointments with accuracy.
Issues such as poly-pharmacy can be tackled using a service like BeAble, to decrease the chance of patient relapse-prevention. Poly-pharmacy is an increasingly common phenomenon that demands attention at clinical level as it can threaten rather than improve a patient’s wellbeing if it’s not managed effectively.
Technologies such as BeAble can ensure best practice in medication management and enhanced self care which are fundamental to independent living with multiple long term condition.

At Bitjam we prefer to choose projects and work with clients in the public sector such as healthcare and education. It’s not only an area we’re more familiar and have more experience in as a collective set of skills, but fields that we have personal preference for as we are contributing towards improving societal communication. 2018 has opened lots of opportunity for us to focus on more data and IoT projects recently, including Staffordshire-based environmental measurement and control company Meaco.
“(Meaco provide) a range of environmental monitoring and control equipment to include data loggers, handheld devices, humidifiers, dehumidifiers and controllers”.
They specialise in providing products and services for high profile national treasures including museums, industrial archives, heritage sites and financial institutions.
Bitjam are to provide a modern solution to replace the current, soon-to-be outmoded hardware and provide effective and highly secure software. To discover more about the project, we’re using our R&D cycle to decipher the requirements, to accurately develop and test hardware and to build appropriate software with scalability in mind.
Currently we’re in the process of getting two units working in the field trial. This has involved the setup of Raspberry Pi’s with software and security configuration, and installing in two different real-situation environments such as a museum or bank.
The project is part of a longer process of working in partnership with Meaco to replace existing software to more modernised and scalable cloud-based technology.
The technology will revolutionise the way that Meaco provide high profile partner services, who deliver a mostly public sector service themselves. This boost of their services has a social benefit so it’s an appealing project to us, as it’s our preference to work in partnership with companies who have a similarly social vision.
We are working in partnership with Meaco director and system creator Michael Hall to innovate the entire system, which has given us a very hands-on experience (something we very much prefer to do at bitjam!). Together we’re working through the challenges of innovating the systems in such a way that business as usual is not affected and that clients are not disrupted during the transition.
